2413 - HEALTH SERVICES - IMMUNIZATION
Immunization Required for School Admittance
The Ohio Department of Health, under the authority granted in R.C. 3313.671, R.C. 3701.13 and R.C. 3323.05 has established the following minimum requirements for students in public schools.
| A. | Four (4) or more doses of DPT or TD (Adult) Vaccine or a combination of these vaccines. * | ||
| B. | Three (3) or more doses of Trivalent Oral Polio Vaccine (TOPV). | ||
| C. | One (1) measles (Rubeola) immunization. | ||
| D. | One (1) Rubella immunization. | ||
| E. | One (1) mumps immunization. |

* An exception to the DTP/TD requirement can be made for students who received their third DTP/TD at age six (6) and above. These students are not required to receive further DTP/TD immunizations. They are required to complete the other immunizations. |
Students who do not meet or exceed minimum immunization requirements should be considered inadequately immunized. Such students should not be admitted to school unless they have received at least one dose of DTP or TD vaccine, one dose of TOPV and immunization against measles, rubella, and mumps.
The State Immunization Law addresses itself to students enrolled in kindergarten through high school. Once immunization-deficient students have been identified they should be followed to ensure that they are receiving the balance of the required immunizations. (All doses of required vaccines that a student has had in the past count toward satisfying the minimum number of required immunizations.)
Parents should be advised that failure to complete the required immunization in a timely manner is basis for excluding students from school; parents should be further advised that readmittance will not take place until proof of additional immunization(s) is submitted.
The required immunizations can be waived if a medical contra-indication exists; a physician's written statement is required. Objection on religious or philosophical grounds is a valid exemption only when a written statement to this effect is signed by a parent or guardian; these parents should be warned that their children are subject to exclusion from school in the event of a school-based outbreak of any of the aforementioned diseases. Evidence that a waiver exists must be kept on file at the school.
Parents are responsible for providing immunization data to the school if this information is not available through other school records.
The School Health Examination Record will be given to the parent/guardian at the time of registration and it is recommended that it be completed with all immunization dates and physical and dental examinations that have been recorded by the specific physicians. It is also noted that a vision examination is also recommended as a visual condition might affect a student's academic performance.
